Medical Weight Loss membership includes everything in Luxe Care, comprehensive initial lifestyle assessment with lab work review and personalized recommendations, Semaglutide medication and supplies shipped directly to you, virtual or in-office physician check-in.

Patient is responsible for the cost of blood work. Lab work done within the previous six months is acceptable for the initial assessment. Lab work is usually covered by your health insurance plan.

Eligibility criteria include a Body Mass Index (BMI) of 30 or higher, or a BMI of 27 or higher with at least one risk factor (ex. high blood pressure, diabetes, high cholesterol).

Luxe Care membership includes 24/7 access to the doctor via secure messaging app, same day/next day appointments, unlimited unhurried office visits, annual comprehensive physical, unlimited telemedicine visits at no charge, medication delivery from Capsule Pharmacy at no charge (within delivery area), access to home lab draws, and discounted aesthetic and wellness services.

Premier Care membership includes everything in Luxe Care, home lab draws (four per year), and 24/7 access to your doctor via personal cell phone.

Elite Care membership includes everything in Premier Care plus six house calls per year (subject to geographic location to our office and during regular business hours).

Items such as laboratory tests, immunizations, diagnostic tests, hospitalizations, specialist visits, and medications are not covered by the membership fee. These services are usually covered by your health insurance plan. We do recommend everyone have health insurance as we are not a health insurance company.

You may see the doctor in the office as many times as medically necessary as a membership patient of our practice. In addition, the doctor is available 24/7 via e-mail, text, and personal cell phone.

We are a membership based practice and prioritize those patients. Self-pay medical patients may be seen on a case-by-case basis. Aesthetic services, wellness services, and Covid testing are available to both membership and non-membership patients.

We do not accept health insurance nor are we a health insurance company. In addition, due to federal regulations, we can not accept patients who are insured by Medicare/Medicaid at this time. For patients paying out of pocket, our self-pay rates are reasonably priced.

We are currently accepting new membership patients. Self-pay patients may be seen on a case-by-case basis. Please note, we reserve the right to decide who we see as a patient in our practice.

We currently do not accept walk-ins. Appointments are required so that we have ample time to verify your demographics.

If your plan is an HMO that requires authorizations to be done only by in-network physicians, we would not be able to do this and you would need to see the primary care doctor to whom you were assigned in order to get an authorization. This typically applies to specialist referrals, imaging studies, physical therapy, home health care, etc.

Payment in full is expected for services rendered. Outstanding balances must be paid off before requesting additional services including appointments or medication refills. We are unable to accept direct family members as new patients of our practice until all outstanding balances from all family members are paid off. We are not a lending institution and, as such, do not offer payment plans. Outstanding accounts will be sent to collections.

We have implemented a policy requiring a credit card to be held on file. Similar to hotels and car rental agencies, you are asked for a credit card number at the time you check in. The information will be held securely until your appointment is complete. At that time, any remaining balance owed by you will be charged to your credit card and a receipt will be sent to your patient portal account. This is an advantage since it makes checkout easier, faster, and more efficient.

There is a $50 administrative fee for any lengthy, time-consuming forms (for example, disability forms) that we may be asked to complete for non-membership patients.

There is a $50 administrative fee for copying, faxing, etc of medical records for non-membership patients who wish to transfer their records to another practice.

These fees are waived for membership patients.

Due to changes in federal and state legislation regarding narcotic pain medications, we do not routinely prescribe narcotics from this office nor do we manage chronic pain.

Refills for controlled substances require a mandatory office visit as regulated by the Florida Department of Health.

When you schedule an appointment with us we set aside enough time to provide you with the highest quality care.

Should you need to cancel or reschedule an appointment please contact our office as soon as possible. This gives us time to schedule other patients who may be waiting for an appointment.

Cancellation requests with less than 24 hours notice will be charged a $50 cancellation fee.

No shows for confirmed appointments will be charged a $50 no show fee.

These fees are waived for membership patients.

We do not provide routine women’s services such as Pap smears or gynecologic exams and procedures.

Please note we are not pediatricians and do not see children. We do not provide routine well-child visits, vaccinations, or sick-child visits.

We do not administer vaccines at this time. They are available at most major pharmacies through your insurance.

House call pricing varies based on location and medical service(s) to be provided. House calls start at $250 per hour not including drive time. Please call to ask about specific pricing.

Yes! We offer telemedicine visits for patients who can’t make it into our office. Virtual visits are included if you are a membership patient of our practice!

For self-pay patients, the cost is $75 for a virtual visit. A credit card is needed to pay for this service.

We perform Covid-19 antibody, antigen, and PCR tests in our office. Results are available the same day in 15-30 minutes depending on the type of test performed. Please note we do not accept health insurance.

Pricing for Covid-19 testing is as follows: $100 for Covid antibody testing, $150 for Covid antigen testing, and $250 for Covid PCR testing. Membership patients receive a discount for Covid testing.

We have partnered with a highly reputable local hospitalist group specializing in Hospital Medicine for your hospitalization needs. This allows us to work closely with your admitting team to coordinate your care and advocate on your behalf.

The valet parking garage is located on North Tampa Street in the Fifth Third Center directly across from the Bank of America Financial Center. Street parking is also available in front of the building on East Kennedy Blvd.
